What tongkat ali is and how it works
Eurycoma longifolia is a slow-growing shrub native to the lowland tropical rainforests of Malaysia and surrounding regions. The root is the medicinal part. It takes 5 to 10 years for a tongkat ali plant to mature enough for the roots to contain therapeutic levels of active compounds.
The primary bioactive compounds are called quassinoids, particularly eurycomanone. These compounds work through a mechanism that is distinct from most "testosterone boosters" on the market.
Most supplements that claim to boost testosterone try to directly stimulate the testes to produce more. Tongkat ali works differently. It acts primarily on sex hormone-binding globulin (SHBG), the protein that binds to testosterone in your blood and makes it unavailable for use. By reducing SHBG levels, tongkat ali effectively increases the amount of free testosterone (the form your body can actually use) without necessarily increasing total testosterone production.
It also appears to modulate cortisol levels, which indirectly supports testosterone. Cortisol and testosterone have an inverse relationship: when cortisol is chronically elevated, testosterone production is suppressed. By helping normalize cortisol, tongkat ali removes a major brake on natural testosterone production.

Testosterone research: what the trials actually found
Let us look at the actual data instead of marketing claims.
This study, published in the Journal of the International Society of Sports Nutrition, gave 63 moderately stressed adults (both men and women) either 200mg of a standardized hot-water extract or placebo daily for 4 weeks. Results: the tongkat ali group showed a 37% increase in testosterone (from baseline) and a 16% decrease in cortisol. Stress profile scores improved by 65%. Importantly, these improvements were most pronounced in participants who started with higher stress levels and lower baseline testosterone.
A 2012 study by Ismail et al. looked specifically at late-onset hypogonadism (age-related testosterone decline) in 76 men. After 1 month of 200mg standardized tongkat ali, 90.8% of the participants showed normal testosterone levels, up from 35.5% at baseline. Their Aging Males' Symptoms (AMS) scores also improved significantly.
A 2014 randomized, double-blind trial by Henkel et al. in the Journal of Ethnopharmacology confirmed significant increases in free and total testosterone in men aged 40 to 65 after 12 weeks of supplementation.
Cortisol and stress: the overlooked mechanism
This is the part most tongkat ali marketing glosses over, but it might be the most important mechanism of all.
The Talbott 2013 study included cortisol measurements for a reason. The researchers hypothesized that much of tongkat ali's testosterone benefit comes indirectly through cortisol reduction. When cortisol drops, the HPA axis stops suppressing the HPG axis (the system that controls testosterone production). It is like releasing a parking brake.
This explains why stressed individuals see the most dramatic testosterone improvements. Their testosterone was not low because of testicular failure. It was low because chronic stress was actively suppressing it. Tongkat ali addresses the root cause (stress hormone imbalance) rather than just the symptom (low testosterone).
This also means tongkat ali has benefits beyond testosterone: improved mood, better sleep quality, reduced anxiety, and enhanced physical recovery are all downstream effects of normalized cortisol.
Male fertility: sperm count and quality
The fertility data on tongkat ali is some of the most impressive in the herb's research portfolio.
A 2010 study published in the Asian Journal of Andrology gave 75 men with idiopathic infertility (unexplained low sperm count) 200mg of tongkat ali extract daily for 9 months. Results: sperm concentration increased by 65.5%, motility improved by 44.4%, and semen volume increased by 18.2%. Remarkably, 14 of the 75 men (18.7%) spontaneously conceived with their partners during the study period. A 2012 follow-up confirmed these findings with similar improvements in sperm morphology.
These are striking numbers for an herbal supplement. The researchers attributed the improvements to tongkat ali's combined effects on testosterone, SHBG, and antioxidant activity (oxidative stress is a major factor in sperm damage).
Can women take tongkat ali?
Yes, and the research supports it for specific applications. The Talbott 2013 study included female participants and found that women also showed improved cortisol levels, reduced stress, and enhanced mood. Testosterone increased in women too, but within the normal female range.
For women, tongkat ali's primary benefits are stress reduction, mood improvement, and potentially enhanced libido (testosterone plays a role in female sexual health as well). It does not cause masculinization at standard doses. The hormonal effects stay within physiological female ranges.
However, pregnant or breastfeeding women should avoid it, as there are no safety studies in these populations.

Dosing, timing, and cycling
Dose
